Job summary

MaziCTools is seeking a qualified RN Case Manager for a fully remote, full-time role. You will assess, plan, implement, and coordinate case management activities telephonically or face-to-face with members, ensuring wellness and regulatory compliance while operating within EST hours.

The position requires an active RN license, 3+ years acute-care experience, 1+ year computer and multi-monitor use, and willingness to obtain additional state licenses. Weekend/holiday work may be required.

Qualifications

• Active, current, and unrestricted RN license in the state of residence.
• 1+ year experience using computers and navigating multiple monitors while talking with a client.
• Willingness to obtain additional state licenses if necessary.
• 3+ years of acute-care RN experience (medical, surgical, pediatrics, ICU, case management, discharge planning).

Responsibilities

• Conduct telephonic and face-to-face assessments of members’ medical needs.
• Plan, implement, and coordinate all case management activities with members.
• Collaboratively develop proactive action plans to address issues and improve short- and long-term outcomes.
• Use clinical tools and data reviews to evaluate member needs and available benefits.
• Refer members to programs offered by the plan sponsor as appropriate.
• Apply clinical judgment to reduce risk factors and address complex health and social indicators.
• Utilize case management processes in compliance with regulatory and company policies.
• Employ interviewing skills to maximize member engagement and discern health status and needs.
• Maintain a private designated workspace free of distractions, with high-speed internet.
