Qualifications

• MS Office proficiency and general computer hardware skills.
• Ability to conduct presentations and training.
• Knowledge of the Americans with Disabilities Act (ADA).
• Strong written and oral communication skills.

Responsibilities

• Develop, maintain, and monitor care plans for high risk and special needs members.
• Work with physicians to determine medical needs.
• Conduct telephone or face-to-face assessments to identify risk factors.
• Foster networks with service and resource providers.
• Create and implement care plans to deliver quality care.
• Maintain care management records and ensure data integrity.
• Educate providers about members' special needs.
• Coordinate with community support and social services.
• Assist in policy and procedure development for care management.
• Communicate with supervisor on departmental issues.
