CVS Health is seeking a remote Case Manager to support Community Care members across the United States. You will assess needs, develop care plans, and coordinate services among providers, payers, and families to promote positive health outcomes.

This full-time role requires an active RN license in FL or OH, 3+ years clinical experience, and strong communication, problem-solving, and digital literacy. Expect a fast-paced environment with HIPAA compliance and collaborative teamwork.

Qualifications

• Active and unrestricted RN license in the state of residence (FL or OH)
• 3+ years clinical experience with Medicare members
• 2+ years case management or home health coordination experience
• Proficient with Word, Excel, Outlook, PowerPoint and basic proprietary apps

Responsibilities

• Coordinate comprehensive care plans across providers.
• Act as liaison with members, families, employers, providers.
• Coordinate case management for catastrophic/chronically ill cases.
• Assess needs and eligibility for benefits; document activities.
• Communicate with stakeholders and provide patient education.
• Monitor progress and advocate for best outcomes.
• Ensure HIPAA compliance and regulatory adherence.
