Job summary

Humana Inc. is seeking a Telephonic Care Manager (Registered Nurse) to support patients transitioning through post-acute care in a remote, full-time role.

You'll coordinate home health, hospice, and palliative care services, acting as a patient advocate to ensure timely referrals and high satisfaction. The role includes potential travel for training and requires an RN license, an associate degree, and preferred BSN with 3+ years post-acute experience.

Qualifications

• Associates Degree required. BSN preferred.
• Compact state RN license required.
• At least 3 years post-acute nursing experience; home health or hospice preferred.
• Knowledge of home health, hospice, and palliative care services.
• Excellent verbal and interpersonal skills; empathy over the phone.

Responsibilities

• Be a CenterWell Home Health representative supporting patients discharged from service or needing post-acute services.
• Make clinical level of care determinations and match needs to service sites; provide alternatives when needed.
• Educate on Home Health, Hospice, and Palliative Care Services and navigate post-acute options.
• Assist in admissions by acting as an ambassador for patients meeting admissions requirements.
• Follow HIPAA privacy and company policies; participate in required training.
