Job summary

Humana Inc in the United States is seeking a Telephonic Registered Nurse Care Manager to support patients transitioning into and out of our services. This remote role reports to the Manager, Care Management and requires knowledge of home health, hospice, and palliative care, with responsibilities spanning care navigation, eligibility assessments, and timely referrals.

Qualifications

• Associates Degree required; BSN preferred.
• We require a compact state RN license.
• At least 3 years post-acute nursing experience; home health or hospice preferred.
• Knowledge of home health, hospice and palliative care services.
• Excellent communication with empathy over the phone.

Responsibilities

• Be a CenterWell Home Health representative supporting discharged patients or those needing post-acute services.
• Navigate care options, post-acute offerings, Medicare coverage and resources.
• Determine clinical level of care based on discussion and records; match to service sites.
• Follow-up and track referral and admission outcomes; ensure patient satisfaction.
• Assist admissions and place patients to appropriate care setting at the right time.
• Adhere to HIPAA, ethics and company policies; participate in mandatory training.
