Job summary

Humana Inc. is seeking a telephonic Care Manager, a Registered Nurse who will support patients transitioning to and from our services. You will determine post-acute needs, educate on home health, hospice and palliative care, and work with the clinical team to ensure timely follow-up and admission decisions.

This is a remote, full-time role requiring an RN license in a compact state, with some travel to Humana offices for training as needed. Salary ranges from $71,100 to $97,800 per year.

Qualifications

• Associates Degree required; BSN preferred.
• Compact state RN license required; additional state licenses may be needed.
• At least 3 years post-acute nursing experience; home health or hospice preferred.
• Knowledge of home health, hospice, and palliative care services.
• Analytical and problem-solving skills.
• Excellent verbal and interpersonal skills; empathetic phone communication.

Responsibilities

• Support patients discharged from service and navigate post-acute options.
• Provide patient education on post-acute care, sites of service, and Medicare coverage.
• Make clinical level of care determinations and match to service locations.
• Assist admissions and advocate for appropriate referrals to maximize timely care.
