Humana Inc. seeks a Telephonic Nurse Care Manager to support patients transitioning between services. This full-time, remote role involves determining level of care, guiding patients through home health, hospice and palliative options, and coordinating with the CenterWell clinical team.

You will work 40 hours per week on a rotating shift, including weekends and holidays as required, with training opportunities and requirements such as HIPAA compliance and compassionate phone communication.

Qualifications

• Associates degree required; BSN preferred.
• Compact state RN license required.
• At least 3 years post-acute nursing experience.
• Knowledge of home health, hospice, and palliative care.
• Excellent verbal and interpersonal skills; empathetic phone communication.

Responsabilités

• Serve as a CenterWell Home Health telephonic nurse liaison for discharged patients and those needing post-acute services.
• Provide patient options and education using various tools, addressing location lookup, directions, and complaints.
• Determine clinical level of care based on discussion, records and data; match to service sites or alternatives.
• Support admissions by guiding patients through requirements and advising on appropriate care settings.
• Ensure adherence to HIPAA, company policies, and mandatory trainings; monitor patient satisfaction and outcomes.
• Assist in referrals and follow-up, aiming for timely responses and admissions.
