Job summary

Sagility is seeking an experienced Registered Nurse to join as a Senior Care Specialist in a work-from-home arrangement. You will conduct telephonic health assessments, develop care plans, and coordinate transitions of care for Medicare, Medicaid, and group health plan members.

The role emphasizes independent work, strong nursing judgment, and collaboration with multidisciplinary teams to improve member outcomes while supporting education and self-management.

Qualifications

• Active, unrestricted RN license in the state of residence.
• Active Compact RN license permitting practice in applicable compact states.
• Minimum of 2 years of clinical nursing experience; 3+ years preferred.
• Ability to work independently in a remote environment with good judgment.

Responsibilities

• Conduct telephonic health assessments for Medicare, Medicaid, and/or group health plan members.
• Develop actionable, individualized care plans based on member needs and risks.
• Establish health coaching relationships and provide education and support.
• Use clinical nursing judgment to assess needs and coordinate interventions.
• Support transitions of care and safe discharge planning with DME and services.
• Help members navigate healthcare benefits and available care options.
• Assess factors affecting a member's health and well-being.
• Collaborate with multidisciplinary teams across disciplines.
• Promote self-management with education and resources tailored to conditions.
• Coordinate care and condition-management activities among team members.
• Document interactions, assessments, care plans, and outcomes accurately.
