Job summary

UnitedHealthcare is hiring a Care Coordinator (RN) to coordinate complex member care across the continuum, including acute, home, chronic, and alternative settings. You will identify health risks, educate members, and collaborate with Case Management, Utilization Review, and Behavioral Health teams.

The role is field-based with remote work flexibility for candidates within commutable distance to Shelby County, TN.

Qualifications

• Current RN license is required.
• 2+ years of clinical RN experience.
• 1+ years of MS Office (Word, Excel, Outlook).
• Excellent organization, communication and time management.

Responsibilities

• Apply clinical knowledge to coordinate case management for Medicare/Medicaid beneficiaries.
• Incorporate evidence-based clinical practices into coordination activities.
• Deliver member-centered self-management support and education.
• Ensure appropriate utilization and consistent application of health benefits.
• Conduct telephonic and in-person assessments per policy and timeframes.
