Job summary

UnitedHealthcare is seeking a Licensed Field Care Coordinator in Tennessee. This field-based role coordinates care for medically complex members, leveraging clinical knowledge to ensure access to services and cost-effective care.

The position supports remote work options within commuting distance to Carroll County. Primary responsibilities include conducting assessments, informing members and providers, and collaborating with care coordination teams to improve outcomes while travel is required

Qualifications

• RN license is required and active.
• 2+ years of clinical RN experience.
• 1+ years of MS Office experience (Word, Excel, Outlook).
• Excellent organizational and time-management skills.
• Reside in or within commuting distance to Carroll County.
• Ability to travel up to 75% within an assigned territory.

Responsibilities

• Apply clinical knowledge to coordinate member care needs.
• Incorporate evidence-based clinical practices into care coordination.
• Deliver member-centered, individualized self-management support and education.
• Ensure appropriate utilization and consistent health benefits application.
• Conduct telephonic and face-to-face assessments per policy and timeframes.
• Serve as member advocate to resolve barriers to care.
• Coordinate community resource referrals to members and providers.
• Develop and maintain member-centered care plans when indicated.
