Job summary

UnitedHealthcare is seeking an RN Clinical Care Coordinator to manage a panel of members with complex medical and behavioral needs. This field-based role involves 80% travel to members in their homes or long-term care facilities across Essex and surrounding counties, NJ.

Responsibilities include comprehensive needs assessments, personalized care plans, care team collaboration, and education to promote self-management and safe transitions.

Qualifications

• Current, unrestricted RN licensure in New Jersey.
• 2+ years of clinical experience.
• 1+ years of MS Office experience.
• Access to reliable transportation and the ability to travel up to 80% within assigned territory.

Responsibilities

• Engage members face-to-face and/or telephonically to complete a comprehensive needs assessment.
• Develop and implement person-centered care plans addressing medical, behavioral, functional, cultural, and socioeconomic needs.
• Partner and collaborate with internal care team, providers, and community resources to implement care plan.
• Provide education and coaching to support member self-management of care needs and lifestyle changes.
• Support proactive discharge planning and coordinate Care Transition after ER, inpatient or SNF admission.
• Advocate for members and families to ensure needs and choices are represented by the health care team.
