Job summary

UnitedHealthcare is seeking a Registered Nurse to manage a panel of members with complex medical and behavioral needs. This role emphasizes field work, home visits, and coordinating care across providers to improve outcomes.

You will assess medical, behavioral, functional and social needs, develop care plans, educate members, and support transitions from hospital to home in a dynamic team setting.

Qualifications

• Current, unrestricted RN license in New Jersey.
• 2+ years of clinical experience.
• 1+ years of MS Office experience: Word, Excel, Outlook.
• Access to reliable transportation and up to 80% travel within territory.

Responsibilities

• Engage members in person or by phone to complete comprehensive needs assessments.
• Develop and implement person-centered care plans for chronic conditions and social determinants.
• Collaborate with care team, providers, and community resources to execute plans.
• Provide education and coaching to support self-management.
• Support discharge planning and Care Transition after ER, hospital, or SNF.
• Advocate for members to ensure needs and choices are respected by the care team.
