Job summary

Hope's Door New Beginning Center (HDNBC) seeks a Rapid Rehousing Case Manager to promote safety, healing, and empowerment for domestic violence survivors through coordinated housing advocacy in Plano and Garland. You will manage caseloads, perform intake, and coordinate rental/utility support with Housing Forward while maintaining client confidentiality and trauma-informed approaches.

Qualifications

• Bachelor's degree in Social Work or related field required.
• Experience with Coordinated Access System (CAS) and Housing Forward processes preferred.
• Experience submitting rental/utility documentation for housing programs.
• Proficiency with Microsoft Office and Salesforce.

Responsibilities

• Support a victim-centered and trauma informed advocacy approach with compassion.
• Collaborate with Housing Forward and landlords to support clients' tenancy; rent is managed by Housing Forward.
• Conduct intake and individualized needs assessments for CAS-referred clients.
• Provide monthly case management, including home visits, office visits, or virtual check-ins.
• Connect clients with supportive services such as counseling, legal advocacy, childcare, transportation, and employment assistance.
• Develop and monitor individualized service plans with clients, focusing on safety, housing stability, and long-term independence.
• Conduct regular home visits at least once monthly; document progress in Salesforce.
• Prepare rental and utility assistance requests; monitor payment schedules and communicate with clients/landlords.
• Provide reports and updates as required by HDNBC and Housing Forward.
