CVS Health is seeking a Practice Transformation Advisor to enhance provider performance, quality of care, and cost efficiency across our care network. You will identify improvement opportunities, partner with large provider groups, and lead education initiatives to align operations with best practices.

The role requires 5–7 years in healthcare quality or provider relations, strong data analysis, and the ability to train teams.

Qualifications

5-7 years of experience in healthcare quality improvement, provider relations, or a related role.
In-depth knowledge of healthcare regulations, clinical operations, quality standards, and performance metrics.
Strong analytical and problem-solving skills with ability to interpret data and identify improvements.
Excellent communication, presentation and interpersonal skills to influence providers and executives.
Experience designing and delivering training programs or educational initiatives.
Proficiency in Excel and data visualization tools.
Familiarity with electronic health record systems and healthcare IT.
Detail-oriented, organized, and able to manage multiple projects.
Willingness to travel up to 30% and work remotely with secure home network.

Responsibilities

Provider Engagement: Establish and maintain engagement with large provider groups that have a membership of 1,000 or greater.
Performance Analysis: Conduct comprehensive analysis of provider performance metrics and clinical operations for VBC readiness and risk assessment.
Provider Education and Practice Transformation: Design and deliver training programs and educational materials for primary care and specialty providers.
Performance Improvement Initiatives: Lead initiatives aimed at improving provider performance.
Quality Assurance: Conduct regular audits, reviews, and assessments of provider practices, documentation and compliance.
Stakeholder Collaboration: Collaborate with internal teams to align provider performance objectives with organizational goals.
Data Management and Reporting: Use analytics tools to collect, analyze, and report provider performance data.
Industry Knowledge and Research: Stay abreast of industry trends and regulatory changes related to provider performance and healthcare quality.
