Job summary

CVS Health is seeking a Practice Transformation Advisor to advance quality and value across our network of providers. You will identify opportunities for improvement, partner with practices to implement quality strategies and measure impact on patient care, cost, and experience.

The role requires 5–7 years in healthcare quality or provider relations, strong data analysis, training design, and ability to work with executives.

Qualifications

5–7 years of experience in healthcare quality improvement, provider relations, or related role.
Strong knowledge of healthcare regulations, clinical operations, quality standards, and performance metrics.
Excellent communication, presentation and interpersonal skills to collaborate with executives and providers.
Proficiency in data management and analysis tools (e.g., Excel).
Familiarity with electronic health record systems.

Responsibilities

Engage with large provider groups (1,000+ members) to drive performance improvements.
Analyze provider performance metrics and clinical operations for readiness and risk.
Design and deliver training programs for primary care and specialty providers and staff.
Lead performance improvement initiatives and monitor outcomes.
Conduct audits and provide feedback to improve documentation and compliance.
Collaborate with clinical, operational, and quality teams to align objectives.
Collect, analyze, and report performance data; present to senior leadership.
Stay informed on industry trends and support health equity initiatives.
