Job summary

CVS Health is seeking a Practice Transformation Advisor to enhance practice performance and quality across our provider network. You will identify improvement opportunities, partner with practices to implement strategies, and drive outcomes in care delivery.

Responsibilities include engaging large provider groups, analyzing performance metrics, designing training programs, and leading improvement initiatives to elevate patient care and cost efficiency.

Qualifications

• 5-7 years in healthcare quality improvement, provider relations, or related role.
• Knowledge of healthcare regulations, clinical operations, quality standards, and performance metrics.
• Strong analytical and problem-solving skills to interpret complex data sets and identify improvement opportunities.
• Excellent communication, presentation and interpersonal skills to collaborate with and influence providers and executives.
• Proven experience designing and delivering training programs or educational initiatives.
• Proficiency in data management and analysis tools, such as Excel or data visualization software.
• Familiarity with electronic health record systems and healthcare IT.
• Detail-oriented, organized, and able to manage multiple projects in a fast-paced environment.

Responsibilities

• Provider Engagement: Establish and maintain engagement with large provider groups (1,000+ members).
• Performance Analysis: Analyze provider performance metrics, clinical operations, and cost of care.
• Provider Education and Transformation: Design and deliver training for primary care and specialty providers.
• Performance Improvement: Lead initiatives, develop improvement plans, monitor progress.
• Quality Assurance: Conduct audits and provide improvement recommendations.
• Stakeholder Collaboration: Align objectives with clinical and operations leadership.
• Data Management and Reporting: Prepare reports and dashboards for leadership.
• Industry Knowledge: Stay updated on regulatory changes and best practices.
