Job summary

CVS Health is seeking a Practice Transformation Advisor to support high standards of practice performance within our provider network. You will identify opportunities for quality improvement and partner with practices to implement strategies that improve care quality, efficiency, and member experience.

This full-time remote role requires strong analytical, training, and collaboration skills. Occasional travel up to 30% may be required to engage with providers and leadership as we advance

Qualifications

• 5-7 years of experience in healthcare quality improvement, provider relations, or a related role.
• In-depth knowledge of healthcare regulations, clinical operations, quality standards, and performance metrics.
• Strong analytical and problem-solving skills, with the ability to interpret complex data sets and identify improvement opportunities.
• Excellent communication, presentation and interpersonal skills to collaborate with and influence providers and executives at all levels.
• Proven experience in designing and delivering training programs or educational initiatives.
• Proficiency in data management and analysis tools, such as Excel or data visualization software.
• Familiarity with electronic health record systems and healthcare information technology.
• Detail-oriented, organized, and able to manage multiple projects simultaneously.
• Ability to work independently, demonstrate initiative, and drive results in a fast-paced environment.
• Position is remote - secure home network required and familiar with Microsoft Office products and VPN - with up to 30% regional travel.

Responsibilities

• Provider Engagement: Establish and maintain engagement with large provider groups that have a membership of 1,000 or greater.
• Performance Analytics: Analyze provider performance metrics and clinical operations for value-based care readiness and risk assessment; identify improvement opportunities and develop data-driven strategies.
• Education and Transformation: Design and deliver training programs and materials for primary care and specialty providers and staff.
• Performance Improvement: Lead initiatives and monitor progress of improvement plans with practice partners.
• Quality Assurance: Conduct audits, reviews, and assessments of provider practices and compliance.
• Stakeholder Collaboration: Work with clinical and operational leadership to align objectives with organizational goals.
• Data Reporting: Prepare reports, dashboards and presentations highlighting KPIs, trends, and opportunities.
• Industry Knowledge: Stay abreast of trends and assist with health equity initiatives.
