Job summary

Carolinas Medical Center is seeking a Care Coordination Specialist in Value Enablement Services – Value Based: Patient Outreach. The position is remote with occasional onsite needs in GA or NC, 40 hours per week, dayshift from 8:00 AM to 4:30 PM, Monday through Friday.

Responsibilities include coordinating intake for new and established cases, verifying health plan eligibility, advancing care plans, generating reports, and training staff on processes.

Qualifications

• Typically requires 5 years of experience in population health management, utilization management, claims, health insurance processing, or care management.
• Knowledge of ICD-10, CPT, and HCPCS coding; familiarity with medical terminology.
• Proficiency in Microsoft Office Suite; strong data management and reporting skills.

Responsibilities

• Coordinate intake of cases and expedite Care Coordination processes.
• Verify health plan eligibility and assess data for care planning.
• Develop and monitor Care Coordination plans and generate reports for stakeholders.
• Train staff and provider office teams on referral/authorization processes and procedures.
• Maintain department records and support population health activities.
