Community Care of North Carolina Inc. seeks a Member Care Coordinator to support non-clinical care management across North Carolina.

You will collaborate with Care Management and QI teams to close care gaps, reduce hospital utilization, and assist with referrals, scheduling, and documentation. The role may be remote within regions or on-site at CCPN practices, requiring travel and strong knowledge of health data systems, MS Office, and HIPAA compliance.

Qualifikationen

• High school diploma or GED required.
• 2-4 years minimum experience in a health care setting.
• Bilingual preferred.
• Knowledge of patient data systems and terminology.
• Proficient with MS Office and internet.
• Ability to coordinate across multiple contacts and regions.

Aufgaben

• Assist with referrals and care gap closure with Care Management team.
• Assist member outreach, engagement, and scheduling.
• Document referrals into the care management platform.
• Access multiple EHRs to obtain and upload documents.
• Coordinate with practices and care team to close gaps.
• Travel within region/state may be required.
