Community Care of North Carolina Inc is seeking a Member Care Coordinator in Cary, NC to support the multi-disciplinary care management team. The role focuses on closing care gaps, reducing hospitalizations, and improving PMPM costs, with potential remote or on-site work within regions.

Responsibilities include outreach, eligibility verification, screenings, referrals, and coordinating access to resources. Strong communication and motivational interviewing skills are essential for success.

Qualificações

• Knowledge of and experience working in patient or clinical data systems.
• Computer skills including various office software and the internet; MS Office preferred.
• Knowledge of state and federal benefits system.
• Excellent communication, both oral and written.
• Proficient Motivational Interviewing skills.
• Organizational and time management skills.
• Sensitivity to diversity of cultures, language barriers, health literacy and educational levels.
• Knowledge of medical terminology.

Responsabilidades

• Receive and document referrals into the Care Management platform.
• Outreach, engagement, and scheduling of members for Care Managers.
• Verify eligibility and demographic information.
• Complete appropriate screenings as needed.
• Assist with referrals on behalf of the Care Management or program team.
• Assist with care coordination and health care system navigation.
• Provide information for access and coordination of resources.
• Identify and address care gaps; support gap closure with providers.
