Job summary

Humana Inc’s CenterWell Senior Primary Care seeks a Physician Collaborator for a fully remote position. The role reports to the Associate Director and supports patients who have had fractures by coordinating with APPs providing wraparound care, with an estimated 5 hours of work per month.

Minimum qualifications include an unrestricted medical license in KY, TN, MO, NC, TX, IN; MD/DO from an accredited university; board certification or eligibility; and at least two years collaborating with

• Graduate of MD/DO program of accredited university

• 2+ years of experience collaborating with mid-levels

Responsibilities

• Report to Associate Director for a fully remote role, averaging 5 hours of work monthly.
