Job summary

Humana's CenterWell Senior Primary Care is seeking a Physician Collaborator for a fully remote role with an average of 5 hours of work monthly. You will collaborate with APPs delivering virtual wraparound services to patients who had fractures in the last six months.

You will report to the Associate Director and work with centers across the CenterWell network to improve care for seniors through value-based models.

Qualifications

• Must have unrestricted medical license in KY, TN, MO, NC, TX, IN.
• Graduate of MD/DO program from an accredited university.
• Board certification or Board Eligible.
• 2+ years of experience collaborating with mid-levels.

Responsibilities

• Collaborate with APPs providing virtual wraparound services for fracture patients.
• Report to Associate Director.
• Remote work with occasional travel to Humana offices for training or meetings.
