Humana Inc. is seeking a Physician Collaborator for a fully remote role reporting to the Associate Director. The position supports virtual wraparound services for fracture patients with about 5 hours of work monthly.

You will collaborate with APPs to deliver care, must hold an unrestricted medical license in KY, TN, MO, NC, TX, IN, and be MD/DO with board certification or board eligibility; at least 2 years’ experience coordinating with mid-levels.

Qualifications

• Must have unrestricted medical license in KY, TN, MO, NC, TX, IN
• Graduate of MD/DO program of accredited university
• Board certification or Board Eligible
• 2+ years of experience collaborating with mid-levels

Responsibilities

• Collaborate with APPs who provide virtual wraparound services to support patients who had fractures in the last 6 months when they cannot see their own PCP.
