Job summary

Humana Inc. is seeking a Physician Collaborator for CenterWell Senior Primary Care in a fully remote role.

You will work with our APPs to support patients who have had fractures in the last six months and cannot see their PCP, reporting to the Associate Director. Responsibilities include collaborating with mid-level providers, maintaining licensure across required states, and contributing to a high-quality, value-based care model focused on seniors.

Qualifications

• Must have unrestricted medical license in KY, TN, MO, NC, TX, IN.
• Graduate of MD/DO program of accredited university.
• Board certification or Board Eligible.
• 2+ years of experience collaborating with mid-levels.

Responsibilities

• Collaborate with APPs who provide virtual wraparound services to support patients who have had fractures in the last 6 months when they cannot get in with their own PCP.
• Work under supervision of an Associate Director and contribute to a high-quality, value-based care model.
