Job summary

Humana Inc. is seeking a Physician Collaborator for a fully remote role with an average of 5 hours monthly workload.

The position reports to the Associate Director and supports virtual wraparound services for fracture care in seniors, collaborating with APPs to deliver high-quality care. Requirements include an unrestricted medical license in KY, TN, MO, NC, TX, IN, MD/DO degree, board certification or eligibility, and at least 2 years of collaboration with mid-levels.

Qualifications

• Must have unrestricted medical license in KY, TN, MO, NC, TX, IN
• Graduate of MD/DO program from an accredited university
• Board certification or Board Eligible
• 2+ years of experience collaborating with mid-levels

Responsibilities

• Collaborate with APPs providing virtual wraparound services to support patients who had fractures in the last 6 months
• Work remotely and report to Associate Director
• Engage in ongoing collaboration to improve care coordination for seniors
