Job summary

Humana's CenterWell Senior Primary Care is seeking a Physician Collaborator for a fully remote role with light weekly hours. You will collaborate with APPs delivering virtual wraparound services for patients who recently fractured, reporting to the Associate Director to coordinate care.

Ideal candidates hold an MD/DO from an accredited university, board certification or eligibility, and unrestricted licenses in KY, TN, MO, NC, TX, IN, with at least 2 years of collaboration with mid-levels.

Qualifications

• Must have unrestricted medical license in the following states: KY, TN, MO, NC, TX, IN
• Graduate of MD/DO program of accredited university
• Board certification or Board Eligible
• 2+ years of experience collaborating with mid-levels

Responsibilities

• Collaborate with APPs who provide virtual wraparound services to support patients who have had fractures in the last 6 months.
• Report to our Associate Director.
• Provide clinical care and collaboration in a largely remote setting.
