Evergreen Nephrology, LLC. is seeking a Chronic Care Management (CCM) Medical Assistant (Care Coordinator) to support proactive care coordination for CKD and related conditions. The role emphasizes outreach, care plan maintenance, and documentation across CCM and EHR platforms.

The position is fully remote but requires working 8:00am–5:00pm in the Central Time zone, with comprehensive benefits and a compensation range of $45,000–$52,000 per year plus a bonus eligibility.

Qualifications

• Certified Medical Assistant certification and active credential.
• 1–3 years of healthcare experience.
• Chronic disease or nephrology experience preferred; strong clinical documentation and communication skills.

Responsabilités

• Support CCM patient enrollment and onboarding activities.
• Verify eligibility based on program guidelines.
• Ensure patient consent documentation is completed prior to outreach.
• Maintain patient engagement and document interactions in CCM and EHR systems.
• Ensure HIPAA/PHI compliance and coordinate care transitions.
