Evergreen Nephrology, LLC. is seeking a Care Coordinator for Chronic Care Management. The role is fully remote, requiring 8:00am–5:00pm Pacific Time availability. You will conduct patient outreach, maintain care plans, and coordinate communication between patients, providers, and partners.

Ideal candidates have CMA credentials, 1–3 years in healthcare, and experience with nephrology care or CKD populations. Strong organization, documentation, and HIPAA compliance are essential for success.

Formación

• Certified Medical Assistant with 1–3 years healthcare experience.
• Chronic disease or nephrology experience preferred.
• Strong clinical assessment, communication, and documentation skills.
• Comfortable working in two systems (EHR + CCM platform) simultaneously.

Responsabilidades

• Support CCM patient enrollment and onboarding activities.
• Verify eligibility based on program guidelines.
• Ensure patient consent documentation is completed prior to outreach.
• Proactively maintain patient engagement regularly.
• Identify inactive or non-engaged patients and escalate appropriately.
• Conduct weekly text & telephonic outreach to enrolled CCM patients.
• Meet Medicare CCM billing engagement time standards.
• Document patient interactions in CCM and EHR systems.
• Maintain audit-ready documentation and HIPAA/PHI compliance.
• Coordinate referrals and follow-up appointments with clinical staff.
