Evergreen Nephrology, LLC is seeking a Telephonic Pharmacy Care Coordinator to support our remote pharmacy program. The role focuses on high-volume outbound outreach, enrollment, consent capture, and EHR documentation for patients with chronic kidney disease and related conditions.

You will work fully remotely by phone, coordinating with a mail-order pharmacy, and ensuring timely access to medications through proactive refill management and care team check-ins.

Qualifications

• CMA/RMA certification is required.
• Experience in a telephonic, high-volume outreach environment is essential.
• Strong interpersonal and communication skills to engage patients over the phone.
• Experience in healthcare, pharmacy, or patient support roles is preferred.
• Proficiency documenting encounters and updates in an EHR/EMR system.
• Knowledge of HIPAA and patient privacy protection.
• Proficiency with MS Office (Outlook and Teams).
• Ability to work remotely with reliable internet and dedicated workspace.

Responsibilities

• Conduct high-volume outbound outreach to enroll patients in Evergreen's pharmacy service.
• Deliver clear messaging using scripts and drive informed enrollment decisions.
• Obtain and document patient consent by phone.
• Update EHR with patient decisions and trigger electronic prescriptions.
• Manage outreach queues and follow-ups to meet enrollment targets.
• Document all activity and maintain HIPAA-compliant records.
• Meet call quality, productivity, and average speed of answer targets.
• Protect patient privacy and PHI according to policy.
