Job summary

Evergreen Nephrology, LLC. is seeking a Telephonic Pharmacy Care Coordinator to support enrollment and ongoing patient engagement for a remote mail-order pharmacy program. You will work primarily from home, contacting patients, obtaining consent, and coordinating care under HIPAA guidelines.

The role focuses on high-volume outreach, enrollment accuracy, and EHR documentation, with a pathway to full-time employment. Central Time availability is required.

Qualifications

• Must have active CMA or RMA certification.
• Experience in telephonic or high-volume outreach settings.
• Strong communication, organization, and privacy skills (HIPAA).

Responsibilities

• Conduct high-volume outbound outreach to enroll patients in the pharmacy program.
• Explain service benefits and guide enrollment via phone.
• Document patient consent and updates in the EHR/EMR.
• Coordinate eRx and pharmacy enrollment steps.
• Maintain call center performance metrics and privacy standards.
