Lumicera is seeking a Pharmacist, Patient Consultation Services to join our telephonic specialty care team. This part-time role supports two primary coverage models and focuses on patient assessments, medication reconciliation, education, and safe medication use for complex therapies.

The ideal candidate holds an ACPE-accredited PharmD or Bachelor of Pharmacy, active pharmacist licensure, and proven experience counseling patients.

Qualifications

• ACPE Accredited School of Pharmacy or PharmD degree required.
• Current, unrestricted pharmacist license in the state of residence or ability to obtain reciprocity within 4 months.
• Experience counseling patients and caregivers on medication therapy and disease state management.
• Experience documenting patient assessments and interventions in pharmacy systems.

Responsabilités

• Conduct telephonic clinical consultations to support safe and effective use of specialty medications.
• Perform patient assessments and medication reconciliation to identify adherence barriers and drug interactions.
• Provide education on disease states, medication administration, storage, and side effects.
• Answer clinical questions from patients and caregivers to aid treatment decisions.
• Document consultations and recommendations per policy and regulatory requirements.
• Maintain productivity and meet service level standards.
• Support quality improvement and report medication errors as needed.
