Job summary

Lumicera is seeking a Pharmacist for Patient Consultation Services to provide telephonic clinical consultations to specialty-pharmacy patients on complex therapies. This part-time role covers assessments, reconciliation, education, and adherence support, interacting with patients, caregivers, pharmacists, technicians, and other health professionals.

Qualifications

• PharmD or ACPE-accredited BPharm degree required.
• Current, unrestricted pharmacist license in the state of residence or ability to reciprocity within months.
• Experience counseling patients on medications and disease state management.
• Experience documenting patient interactions and adherence strategies per standards.

Responsibilities

• Conduct telephonic clinical consultations with specialty-pharmacy patients.
• Perform patient assessments and medication reconciliation.
• Educate on disease management, administration, adherence, and side effects.
• Identify potential medication-related concerns and intervene as needed.
• Document consultations and actions in pharmacy systems per policy.
• Maintain productivity, quality, and service levels; support safety initiatives.
• Stay current on specialty meds and evolving therapies; comply with laws and standards.
• Collaborate with patients, caregivers, pharmacists, and clinicians.
