Job summary

ComplexCare Solutions (CCS) specializes in home and virtual health assessments for health plans nationwide. This role involves performing comprehensive one-hour NP assessments in the member’s home, documenting findings, and coordinating with PCPs and plans to close care gaps.

The Nurse Practitioner will work per diem across CT with opportunities for telehealth visits. Requires active NP licensure, ANCC/AANP board certification, and HIPAA compliance; in-home visiting and remote care experience is

Qualifications

• Active unencumbered NP license
• ANCC or AANP board certification in Family, Adult, Gerontology or Emergency Medicine
• Maintains current CPR certification
• Vaccination/testing compliance prior to hire (Hepatitis B, Flu, MMR, Tdap, Varicella, TB)

Responsibilities

• Conduct comprehensive in-home member assessments.
• Document assessment findings, recommendations, and care plans.
• Coordinate with PCP and health plan for follow-up and services.
• Close gaps in care and address chronic condition management.
• Maintain timely documentation in medical records.
• Comply with CMS/DHHS, company policies and client requirements.
