Qualifications

• Active un-encumbered nursing license.
• ANCC or AANP board certification as NP or CNS.
• CPR certification current.
• Vaccination/testing requirements: Hepatitis B, Flu, MMR, Tdap, Varicella, TB.
• Home Health experience is a plus.

Responsibilities

• Conduct comprehensive member assessments and document findings.
• Develop evidence-based treatment plans for chronic conditions.
• Coordinate with physicians and health plans for care gaps.
• Perform and document Point of Care testing results.
• Maintain HIPAA-compliant patient records and communications.
