Job summary

Phoenix Children's Hospital, Inc. is seeking a Care Coordinator to provide comprehensive care coordination for a large pediatric population across Maricopa County.

The role supports families by connecting them to medical, behavioral health, school, community, and government resources, ensuring continuity and quality of care. This remote position (Arizona residents only) offers a standard 8-hour shift with occasional in-office expectations.

Qualifications

• Two or more years of experience in healthcare support roles.
• Excellent phone-based communication and patient engagement skills.
• Proven ability to multitask in a fast-paced environment.
• Highly organized with strong time management and follow-through.
• Self-motivated with ability to work independently.
• Bilingual in Spanish (preferred).

Responsibilities

• Conduct proactive outreach to patients and families to assess care needs.
• Coordinate services across medical, behavioral health, school systems, and community resources.
• Manage incoming referrals and ongoing care coordination activities.
• Support population health initiatives (well-child visits, immunizations, chronic conditions).
• Build rapport with patients and families telephonically and ensure timely follow-up.
