Clarity Pediatrics, Inc. is seeking a Nurse Care Coordinator (RN) to join our remote interdisciplinary care team.

You will coordinate clinical appointments, educate families, manage medication authorizations, and serve as the primary contact between families, providers, and pharmacies for children with chronic conditions. This fully remote role requires an active California RN license, strong tech skills with EHR systems, and excellent communication.

Qualifications

• Active California RN license in good standing (Associate's, Bachelor's, or Master's degree in Nursing accepted)
• Proficiency with multiple technology platforms and electronic health record (EHR) systems
• Strong organizational, communication, and interpersonal skills
• Ability to manage competing priorities and work collaboratively in a remote, virtual environment
• Comfort working with diverse pediatric populations
• Startup mindset and willingness to iterate on care models
• Reliable internet connection and a dedicated remote workspace
• Bilingual: English/Spanish

Responsabilités

• Coordinate, schedule, and reschedule clinical appointments across the care team
• Provide clinical updates to referring physicians and coordinate lab work
• Educate families on chronic condition management and treatment plans
• Initiate medication prior authorization requests and coordinate refills with pharmacies
• Document clinical interactions in EHR and participate in team meetings
