Job summary

Phoenix Children's Hospital is seeking a Case Manager to provide comprehensive care coordination and discharge planning for pediatric patients. You will assess plans of care, establish estimated length of stay, and collaborate with the clinical team to ensure optimal patient outcomes.

Role requires RN licensure in Arizona, with 1 year of care coordination experience preferred; Master’s in Nursing is desirable. Strong communication and EMR documentation skills are essential.

Qualifications

• Bachelor's degree in Nursing or an equivalent combination with RN licensure.
• Minimum 1 year of care coordination and discharge planning experience.
• Master's degree in Nursing preferred; CCM certification beneficial.

Responsibilities

• Coordinate care for a defined patient population to optimize discharge.
• Develop and implement discharge plans with the care team using MCG guidelines.
• Document care plans and communicate with families and payers as needed.
• Collaborate with multidisciplinary teams to ensure appropriate level of care.
