Qualifications

• Active social work or healthcare licensure (BSW/MSW/LPN/RN/CMA or equivalent).
• Two+ years in geriatric behavioral health or older adult care preferred.
• Familiarity with EHR and telehealth platforms; HIPAA compliant.
• Fluent English; additional languages a plus.

Responsibilities

• Conduct psychosocial assessments to identify patient needs and supports.
• Collaborate with care team to plan interventions addressing social factors.
• Connect patients with community resources and benefit programs.
• Educate patients and caregivers on navigating healthcare systems.
• Advocate for patient goals and coordinate care with providers.
