Qualifications

• Associate's degree in Healthcare or related field or equivalent experience.
• Minimum two years in a directly related healthcare role.
• Proficiency with Microsoft Office and web-based technologies.
• Strong outbound calling experience.
• Ability to work independently in a productivity-based environment.
• Tech-savvy and able to navigate multiple systems.
• Comfort with learning proprietary platforms.
• Basic Excel (pivot tables, formulas, spreadsheets).
• Reliable high-speed internet per company specs.
• Flexible schedule within assigned market hours.
• Ability to pivot between priorities quickly.

Responsibilities

• Conduct outbound calls to members to assess health needs and determine program eligibility.
• Assist patients with scheduling appointments and accessing community resources.
• Collaborate with patients, providers, and staff; document all activities.
• Communicate available care coordination services to patients and caregivers.
• Screen for physical/behavioral health needs to refer to appropriate professionals.
• Provide data to providers, patients, and families.
• Participate in educational programs to maintain skills.
• Perform miscellaneous duties as assigned.
