TeleMate Health INC. seeks an experienced TeleMate Health Social Worker to support patients’ health and wellness through resource coordination, education, and advocacy from a fully remote setting.

In collaboration with the care team, you will help patients overcome barriers to care, promote engagement, and improve quality of life. The role emphasizes compassionate, patient-centered care and evidence-based practices.

Qualifications

• Active RN or LPN license required.
• Residency in one of the eligible states listed in the posting.
• Minimum 2 years of experience with geriatric behavioral health or related populations.
• Experience with EHR and telehealth platforms.
• Strong communication and organizational skills required.
• Ability to work independently within a multidisciplinary team.
• HIPAA-compliant remote work environment or workspace.

Responsabilités

• Perform psychosocial assessments to identify patient needs and supports.
• Collaborate with care team to create individualized plans addressing social and environmental factors.
• Connect patients with community resources, benefits, and support services.
• Educate patients and caregivers on navigating healthcare systems and resources.
• Advocate for patients to ensure care aligns with goals and preferences.
• Coordinate with healthcare providers and agencies to promote care continuity.
• Document all interactions and interventions accurately in the EHR.
• Monitor progress and follow up to ensure needs are met.
• Escalate urgent psychosocial or behavioral health concerns to the team.
