Qualifications

• Associate’s degree in Healthcare or related field
• Minimum two (2) years of experience in a directly related position in the healthcare industry
• Proficiency with Microsoft Office applications and web-based technologies
• Basic Excel proficiency (pivot tables, formulas, project-based spreadsheets)
• Flexible schedule within assigned market hours
• Previous experience coordinating care for members
• Familiarity with federal and state laws and requirements related to healthcare and health information protection
• Basic Life Support (BLS), CMA, CNA, EMT
• Excellent problem-solving skills and ability to work independently
• Bilingual (strongly preferred)
• Experience in customer service environments (call center acceptable)

Responsibilities

• Conduct outbound calls to members to assess health needs and eligibility for care coordination programs
• Assist patients with barriers to care such as scheduling appointments and accessing resources
• Collaborate with patients, network providers, referral coordinators, and hospital staff; document activities
• Communicate available care coordination services to patients and caregivers
• Screen for physical and behavioral health needs to refer to appropriate professionals
• Provide meaningful data to providers, patients, and families
