TeleMate Health INC. seeks an experienced TeleMate Health Social Worker to support patients’ health and wellness through resource coordination, education, and advocacy from a fully remote setting.

In collaboration with the care team, you will help patients overcome barriers to care, promote engagement, and improve quality of life. The role emphasizes compassionate, patient-centered care and evidence-based practices.

Qualifications

• Active RN or LPN license required.
• Residency in one of the eligible states listed in the posting.
• Minimum 2 years of experience with geriatric behavioral health or related populations.
• Experience with EHR and telehealth platforms.
• Strong communication and organizational skills required.
• Ability to work independently within a multidisciplinary team.
• HIPAA-compliant remote work environment or workspace.

Responsibilities

• Perform psychosocial assessments to identify patient needs and supports.

Position Overview

As a TeleMate Health Social Worker, you will play a vital role in supporting patients’ overall health and wellness through resource coordination, education, and advocacy. Working closely with the care team, you’ll help patients overcome barriers to care, promote engagement, and improve quality of life. Key Responsibilities

• Conduct basic psychosocial assessments to identify patients’ needs, challenges, and available supports.
• Collaborate with the care team to create individualized plans addressing social and environmental factors that impact health.
• Connect patients with appropriate community resources, benefit programs, and support services.
• Provide education to patients and caregivers on navigating healthcare systems and available resources.
• Serve as a patient advocate, ensuring care aligns with the patient’s goals and preferences.
• Communicate with healthcare providers and agencies to promote coordination of care.
• Accurately document all interactions and interventions in the electronic health record (EHR).
• Monitor patient progress and follow up to ensure needs are met.
• Identify and elevate urgent psychosocial or behavioral health concerns to appropriate team members.

Qualifications and Skills

• Current, active professional licensure in one of the following (or similar):
  • Licensed Professional Nurse (LPN)
  • Registered Nurse (RN)
• Must reside in one of the following states:
  • Alabama
  • Georgia
  • Kentucky
  • Mississippi
  • Pennsylvania
  • Tennessee
• Minimum of two years of experience working with geriatric behavioral health, chronic disease management, or older adult populations preferred.
• Strong understanding of behavioral health conditions, community resources, and social determinants of health.
• Excellent communication, assessment, and organizational skills.
• Ability to work independently while collaborating effectively within a multidisciplinary team.
• Comfortable creating new workflows and implementing process improvements.
• Experience with electronic health records (EHR) and telehealth platforms preferred.
• High‑speed internet connection and a private, HIPAA‑compliant workspace.
• Fluency in English required; additional languages are a plus.
• Must reside in the United States and be legally authorized to work for any employer.
• Must successfully pass a background check.
