Job summary

Lorian Health is seeking a Patient Care Coordinator to support intake of referrals and manage clinical field staff schedules. You will assist with admissions, discharges, resumption of cares, recertification visits, and routine follow-ups, while answering the phone.

You will collaborate with physicians and other care team members to ensure timely, accurate patient data entry and compliant documentation within Medicare guidelines.

Qualifications

• Working knowledge of Medical terminology.
• Proficient with Microsoft Office and EMR systems (HCHB preferred).
• Professional and customer-service driven phone and communication skills.
• Effective communication with patients, doctors, and staff.
• Strong ability to organize and prioritize workload with referrals.
• Flexible in fast-paced, team-oriented environment.
• Maintains patient privacy under HIPAA and state laws.

Responsibilities

• Handle intake of new referrals and maintain clinical field staff schedules.
• Liaise between Lorian Health and referral sources for smooth processing.
• Ensure patients are seen as ordered within 48 hours.
• Enter patient information into HomeCare HomeBase to meet Medicare requirements.
• Collaborate with sales and clinical management on new referrals.
• Follow up on holds and status changes in patient episodes.
• Perform other duties as assigned.
