Job summary

PAFC is seeking a Patient Engagement Representative in Phoenix, AZ to proactively engage patients and guide them toward scheduling preventive and follow-up care. You will use consultative communication to address barriers, reinforce the value of recommended services, and help patients establish ongoing care relationships.

Ideal candidates will have 2–3 years in healthcare outreach, strong English/Spanish communication, and proficiency with CRM and EHR systems.

Qualifications

• Two to three years of experience in healthcare outreach, patient engagement, appointment scheduling, care coordination, inside sales, customer retention, or similar.
• Experience conducting proactive outbound outreach and working toward appointment scheduling, conversion, retention, or engagement goals.
• Healthcare experience is preferred due to empathic patient communication.

Responsibilities

• Conduct proactive outbound outreach to targeted patient populations via phone, text, and other approved channels to promote services and schedule appointments.
• Utilize consultative techniques to understand patient needs and motivate scheduling of care.
• Address patient objections, concerns, and barriers through questioning, listening, education, and solutions.
• Convert outreach into scheduled appointments by communicating availability and importance of timely care.
• Perform follow-up outreach to improve engagement and conversion.
• Engage overdue or at-risk patients to strengthen relationships with providers and the organization.
• Assist with PCP assignment or reassignment as applicable.
• Accurately document outreach attempts and patient interactions in designated systems.
• Use CRM, scheduling systems, EHRs, and dialers to manage outreach and patient interactions.
• Manage campaigns to meet productivity, quality, and engagement targets.
• Apply communication strategies based on patient population and history.
• Maintain knowledge of services, scheduling, and care recommendations.
