Job summary

Imagine Pediatrics, a tech-enabled pediatric group, seeks a Community Health Worker to re-engage patients and families in care across Southern New Jersey. This remote role requires travel throughout the area and collaboration with a multidisciplinary team to reconnect patients who paused onboarding or stopped using services.

You will build relationships in communities, provide onboarding support, educate families on services, and connect them with local resources, with a focus on improving

Qualifications

3+ years of healthcare experience in patient-facing roles.
CHW certification is required.
Valid driver’s license and ability to travel frequently.
Experience coordinating care and addressing SDOH needs.

Responsibilities

Re-engage patients and caregivers by meeting them where they are.
Assist with program re-enrollment, onboarding, and app setup.
Perform outreach to caregivers to re-engage families in care.
Educate families about Imagine Pediatrics services and local resources.
Coordinate with Behavioral Health for discharge planning and follow-up.
Identify barriers to access and support care coordination in the community.
