Job summary

Imagine Pediatrics is a tech-enabled pediatric group focused on re-engaging children in care across Southern New Jersey. The role sits within the Patient Engagement Team and involves in-person community outreach, onboarding support, and coordination with Behavioral Health to ensure seamless care connections.

You will conduct telephonic outreach, educate families about services, and partner with caregivers and facility staff to overcome engagement barriers, traveling frequently within the region.

Qualifications

• 3+ years of experience in a healthcare setting (patient-facing hospital or facility role).
• Bilingual (Spanish) required.
• Ability to travel frequently.
• Manage multiple tech platforms.

Responsibilities

• Re-engage patients who were enrolled but did not complete onboarding or have not actively utilized services.
• Provide hands-on support for onboarding and mobile app setup.
• Telephonic outreach to caregivers to re‑engage children in care and support onboarding.
• Educate patients and caregivers on Imagine Pediatrics services and available community resources.
• Maintain knowledge of local community resources and connect families to appropriate services.
• Conduct community events and facility rounding to engage eligible patients.
• Collaborate with Behavioral Health teams to support discharge planning and ongoing care connections.
• Partner with caregivers, families, and facility staff to identify barriers to engagement and enable access to care.
• Work with Outreach Specialists and Registered Nurses to support patient re-engagement goals.
• Support non-clinical care coordination in the community (connecting patients to primary care, addressing SDOH needs).
