Job summary

Imagine Pediatrics is a tech-enabled pediatric medical group that supports children with special health care needs. The Patient Engagement Associate II will enroll and onboard children and caregivers into the program, collaborate with care teams, and address barriers to enrollment.

This is a remote role with local travel as needed, offering an hourly rate of $24–$26 plus an annual bonus and a competitive benefits package.

Qualifications

• 2+ years in healthcare care coordination, community health, patient engagement, customer service, or sales.
• Proven success in a centralized contact center environment.
• Experience with resourcing within a patient or family-facing role and navigating social service agencies.
• Strong documentation skills; prior experience with a CRM platform is a plus.
• Excitement to be part of a tight-knit team with shared accountability for enrollment; excellent collaboration skills.
• Stellar active listening and communication skills; ability to tailor value proposition to various audiences with different needs and overcome more complex objections.
• Strong ability to build trust & relationships with caregivers & other healthcare organizations.
• Autonomy; reliable internet; access to transportation for field when necessary.

Responsibilities

• Conduct thorough research on attributed patients to understand their healthcare needs and enrollment barriers.
• Perform telephonic outreach to caregivers of attributed patients to enroll their child and onboard.
• Reach out to other healthcare professionals involved with attributed patients and their caregivers (DME, home health, pharmacies, pediatricians).
• Leverage relationships with Imagine teammates and external providers to support ongoing engagement and care coordination.
• Handle inbound calls from caregivers, health plan service coordinators, and hospital discharge planners during enrollment.
• Develop deep knowledge of the SSI program and outreach to eligible caregivers.
• Collaborate with Provider Engagement Specialists for embedded outreach at PCP offices or facilities.
