Job summary

somewhere is seeking a Patient Care & Program Coordinator to own the 12-month patient journey in a remote, part-time capacity. This non-clinical role coordinates appointments, lab kits, and adherence to physician protocols, while maintaining patient confidentiality and high-touch service.

You'll manage 5–20+ active monthly patients, track PROs, and act as the administrative bridge between patients and functional medicine physicians.

Qualifications

• 2–4+ years in Patient Care coordination or similar roles.
• Ability to read and discuss medical protocols from an administrative perspective.
• Excellent written and spoken English with high emotional intelligence.
• Strong organizational skills for multi-month, recurring tasks across large patient rosters.
• Clear understanding of the line between administrative support and clinical medical advice.

Responsibilities

• Coordinate the 12-month patient journey and appointment tracking.
• Handle lab kit logistics and coordinate with partners.
• Monitor protocol adherence and collect Patient-Reported Outcomes (PROs).
• Escalate clinical questions appropriately to attending physicians.
• Maintain high-touch, confidential patient communications.
