Job summary

WOW Remote Teams is seeking a Patient Coordinator to support a high-volume virtual care operation, handling outreach, insurance eligibility verification, referral processing, and accurate medical record documentation.

You will contact eligible patients, explain care management programs (CCM, RPM, TCM, BHI), verify coverage, and update Athenahealth records while maintaining HIPAA-compliant documentation and meeting daily referral goals.

Qualifications

• U.S. Medical Assistant (MA) certification is required.
• Experience with another EMR/EHR platform is a plus.
• Knowledge of CCM, RPM, TCM, BHI, telehealth, or chronic care programs is preferred.
• Experience working remotely with a U.S.-based healthcare organization is a plus.

Responsibilities

• Conduct outbound calls to eligible patients and introduce available virtual care services.
• Explain CCM, RPM, TCM, BHI and other care management programs clearly and compassionately.
• Educate patients on program benefits and enrollment details.
• Obtain and document patient consent according to Medicare requirements.
• Update patient demographics, insurance information, referrals, and orders in Athenahealth.
• Review referrals for completeness and accuracy.
• Maintain HIPAA-compliant documentation and protect patient information.
