Job summary

Pathways Healthcare is seeking a patient communications specialist to join our remote team. You will interact with patients by phone, educate them using data from their electronic medical records, and coordinate with our care team to ensure a seamless experience.

The role offers a full-time schedule (Monday–Friday), initial in-office training, and the possibility of occasional meetings at the Westwood, MA office. Join a leading home health and hospice provider.

Responsibilities

• Interact with patients in a positive and professional manner via telephone.
• Handle multiple calls while maintaining exceptional patient experience.
• Educate patients using data from their electronic medical records.
• Communicate with our healthcare team and act as a liaison between patients and staff.
• Embrace new technologies and continuous improvement to provide our patients with the best possible experience.
• Perform administrative functions as necessary.
• Respond to and reply to requests for information.
• Maintain strict compliance to HIPAA and patient privacy.
• Perform other related job duties as assigned.
